North Central District Health Department is vaccinating anyone 60 & older that live or work in our district at this week’s vaccination event on 03/04/2021. Please share this information with your contacts!
 
Go to https://www.ncdhd.com/covid_vaccine to schedule an appointment for covid-19 vaccine if you are 60+ AND live or work in Shelby, Henry, Spencer, or Trimble Counties.
 
The Vaccine Appt. will be in Shelbyville at NCDHD. Please wear a mask, bring a photo ID & consent form filled out.
If you cannot print the consent form there will be one provided to you at the vaccine appt. To fill out this form you need your health insurance card and social security #. 
 
We are asking members to take a moment and please fill out a survey for U of L Health Shelbyville Hospital

SCHOOL UPDATE
  • Executive Order signed to allow all in person school starting March 1, 2021

  • Third round of P-EBT for students set to be dispersed March 15th
  • If school has been all virtual, students will receive $136.40 each
  • If school has been hybrid schedule, students will receive $81.84 each

VACCINE PHASES
The planned vaccination phases are:
  • Phase 1a: Long-term care facilities, assisted living facilities, health care personnel
  • Phase 1b: First responders, Kentuckians age >= 70, K-12 school personnel, CHILD CARE WORKERS
  • Phase 1c: Kentuckians age >= 60, anyone older than 16 with U.S. Centers for Disease Control and Prevention (CDC) highest-risk conditions for COVID-19, all essential workers
  • Phase 2: Age >= 40
  • Phase 3: Age >= 16
  • Phase 4: Children under the age of 16 if the vaccine is approved for this age group (estimated to comprise 18% of Kentucky’s population)
